Transcript Fees & Procedures
Requests must be made in writing with the student's name, social security number or holyfamily ID number, last date of attendance, destination of transcript with full address, payment and signature of student.

download the Request for Transcript Form. Forms are available in the Office of the Registrar HFH Room 205.
FEES:
$3 per electronic transcript
$3 per transcript if request is sent in by fax / credit card
$4 per paper transcript
$10 same day service (pick up)
$25 same day service and Fed Ex
Transcripts will not be released if the student has any outstanding debts in the Business Office or Library.
Official transcripts are mailed or emailed directly to the receiving agency. Student copies are provided to the student for personal use and will not have the official University stamp or Registrar's signature.
Transcripts will not be released to third parties without the written consent of the student.
